Outcomes from the first dedicated diagnostic and interventional nephrology (DIN) service in a UK renal unit

Background: We report the clinical outcomes, operational and training model from first diagnostic interventional nephrology (DIN) department in a UK renal unit. Methods: Patient outcomes were evaluated for an array of therapeutic procedures performed at DIN unit, SDIN (the Sunderland Diagnostic Intervention Unit), within year its establishment. Data was retrospectively collected period beginning 1st October 2019 to 2020 patients who underwent following procedures: ultrasound guided biopsy, Tunnelled Dialysis Catheter (TDC) insertion exchange, Peritoneal (PD) catheter insertion/exchange, Areteriovenous Fistula (AVF) Point Care Ultrasound (POCUS). These figures compared cohort one-year pre-SDIN, between 2018 31st September 2019. All results are expressed as mean percentages unless otherwise specified. Results: Renal Biopsy: 104 biopsies with improvement median waiting time 12 7 days 98.4% being diagnostic. Tunnelled Catheters: 99 TDCs inserted or exchanged catheters remaining place duration 156 days. incidence 2 infections per 1000 90-day observation period. Peritoneal dialysis catheters: 16 PD they remained average 153 Eleven (69%) had no complications 28-day period, 3 (19%) required manipulation. AVF POCUS: 279 AVF POCUS scans during period. The referral scan reduced 35 Conclusions: A comprehensive service leads significant improvements training, patient would be ideal wider adaptation across units.
